PERSONAL INFORMATION
Kindly inform the clinic of any changes to your personal information, including phone numbers and addresses. This is crucial as such updates can delay patient communication.
Arrival Time
Please show your ALBERTA HEALTH CARE CARD at each visit.
We kindly ask that you arrive at least 10 minutes before your scheduled appointment time.
This will give you the opportunity to complete any forms, update your contact information, or the nurse take any necessary vitals (such as blood pressure, height, and weight).
**No Show Policy**
We recognize that unexpected situations can arise from time to time.
When you make an appointment and fail to attend, it results in an available slot that could have been utilized by another patient requiring care.
If a patient misses three appointments, a $55 no-show fee will be applied.
**Cancellations**
Patients are required to give a minimum of 24 hours' notice for cancellations, which must be made by PHONE at 587 797 4088.
This policy is established to honor the time that could have been allocated to another patient. Thank you for your understanding.
